Dementia Workshop - 16th October - Book your places


Harrow CCG/LBH and Public Health are holding a Dementia workshop. 

They are inviting Carers/Users and Professional – organisation leads to our Dementia workshop on 16th Oct.

The max from each organisation will be 5 so we have an even participation on each table.

The workshop is planned to take place on Wednesday 16th October 2013 from 1 pm to 4.30 pm at the Harrow Arts Centre in Hatch End.

Invitations will be sent to a full range of stakeholders with approx. 100 people in attendance from across service users and carers, voluntary sector, health and social care representatives

We have decided to adopt a World Café type of approach to the workshop, with an integrated pathway of care focus

Five tables will be set up as follows:

a)      Early Recognition and Diagnosis

b)      Pathway to Primary and Community services

c)       Pathway to Acute and Intermediate care services

d)      Pathway to End of Life services

e)      Central table on Information and Advice

This is a critical session for taking forward the Dementia Strategy in Harrow and developing an effective Action Plan. Your involvement is really important in ensuring that integrated dementia services are taken forward.
