Embracing self care for life – Self Care Week 13-19 November
By next April, 25,000 people will have received tailored self care support from the vanguards – helping patients, many with long-term conditions, to better manage their own health and wellbeing. With lessons learned from vanguards, self care is being embraced in wider health service planning across larger populations but it is still operating 'at the margins’ of care.
Dr Pritti Mehta, the new care models lead forself care, will take part in a twitterchat on 13 November, 12-1pm at #SCWchat. You can read Pritti’slatest blog.
On 16 November, there will be two webinars:
· 11.30am-12.30pm: Dudley MCP vanguard will talk about the impact they are having by using the Patient Activation Measure. Book your place through email@example.com
· 1- 2pm: Understanding and using the patient activation measure in the NHS, hosted by the Health Foundation. Register for a place.
Changing the core of the NHS – The Economist’s view
The Economist examines how vanguards are changing the core of the NHS. The article cites Better Care Together, the Morecambe Bay vanguard, saying that by working in a less fragmented way, using innovations like hospital video links and drop-in cafes, patient care is being improved, resulting in fewer admissions to A&E.
A small step for primary care, a giant leap for patients
New Care Models Programme Director Louise Watson features in Practice Management Today talking about the value of practice managers in the transformation of care.
In the article, Louise says: “Through Sustainability and Transformation Partnerships – and emerging accountable care systems – the work of the new care models programme will continue to influence and inspire change – change built on excellent primary care in open collaboration with other partners who are all prepared to share clinical and financial risk to achieve better population health outcomes.”
Making a difference to quality of life
At East and North Hertfordshire vanguard’s showcase day, New Care Models care homes lead William Roberts talks about the programme’s work to improve the quality of life for residents of care homes and people supported in extra care living schemes. East and North Herts is one of six vanguards delivering enhanced health in care homes. Watch the film.

Cancer drugs project delivers benefits for patients
The Cancer Vanguard has developed a toolkit that helps the NHS to deliver specialist drugs out of hospital for breast cancer patients. Normally, Denosumab, is given to patients in chemotherapy units in hospitals. It helps to prevent serious bone problems in breast cancer patients.
The toolkit was developed jointly with the Cancer Vanguard following the launch of the Pharma Challenge which invited commercial companies to pitch ideas to improve the supply of cancer medicines.
