Dear all,

It is an exciting time for diabetes care in North West London with the 8 Clinical Commissioning Groups coming together to improve services and care.

What is happening in Harrow?

NHS Harrow Clinical Commissioning Group has worked on a strategy looking at how to deliver the best care for local people with diabetes and to prevent type 2 diabetes.  This links in to the wider North West London Diabetes Transformation Programme. The Harrow Diabetes Strategy Implementation Group meets every 6 weeks to ensure this strategy is implemented.  The group is made up of people with diabetes and carers, commissioners, health and social care professionals, public health representatives, and voluntary sector representatives.

Would you like to be a patient representative on this group?

We are looking for a representative with diabetes to gather views of other people with diabetes and attend meetings to ensure these views are included in the way diabetes care is developed.

Not done anything like this before?  Don’t let that put you off! You will receive a briefing and support from the Clinical Commissioning Group and Diabetes UK. (PLEASE SEE DOCUMENTS ATTACHED BELOW)

 I am interested, what next?

 Spaces on the group are limited so we will ask you to tell us a bit about yourself in the Expression of Interest form and meet with us informally to help us find the right people for the role.  The meeting will also be an opportunity for you to work out if the role is right for you.
